Contest Rules
1. Contest ends August 15, 2005. All surveys must be postmarked or completed online no later than midnight on August 15, 2005.
2. Only one entry per person will be accepted. Winning entries will be drawn at random from all valid entries received. The Gaming Guide is not responsible for late, misdirected, lost, or mutilated mail or entry forms. If entry is illegible or does not supply all information deemed necessary to declare a winner, entry will be deemed invalid and another entry will be selected.
3. Surveys must include name, address, and contact details for prizes to be sent to winners. The poll and survey form must be completely filled out to be eligible for any prizes.
4. Contest is open to all legal residents of Southern California. All contestants must be at least 21 years of age.
5. Winners’ names will be posted on www.TheGamingGuide.com no later than September 1, 2005. Winners of the Super Jackpot and Jackpot prizes will be notified by telephone or e-mail.
6. Prizes are non-transferable and substitutions of cash equivalent will not be offered.
7. Each winner agrees that the Gaming Guide can use his/her name and likeness for promotional purposes without
compensation.
8. Winners must provide valid ID and Social Security number to claim prize. Winners must sign a release form provided by the Gaming Guide to obtain prizes valued over $599.00.
